REGISTRATION OF CERTIFICATE OF LIVE BIRTH (PROMPT)

  • Requirements: (For Legitimate Children)
    • 1 Photocopy of Marriage Contracts of Parents

DELAYED REGISTRATION OF CERTIFICATE OF LIVE BIRTH

  • Requirements: At least two documentary evidence showing the name of Child, Date & Place of Birth & Name of Parents:
    • Original Baptismal Certificate
    • Voter Certification
    • Medical/Service Record
    • Income Tax Return (ITR)
  • Other Requirements:
    • Negative Certification issued by PSA
    • Joint Affidavit of Birth by Two Witnesses
    • Valid IDs of Applicants/Parents
    • Special Power of Attorney, for Authorized Representatives
    • If not married, Affidavit of Acknowledgement/Admission of Paternity
    • For those born and to be registered outside Guiuan, execute an affidavit of Out-of-town reporting attested by two (2) witnesses of the birth event
    • Special Power of Attorney, for Authorized Representative

APPLICATION OF MARRIAGE LICENSE

  • Requirements:
    • Certificate of Live Birth
    • CENOMAR issued by PSA
    • Identification Cards
    • Parental Advice/Consent
    • Certificate of Attendance to Pre-Marriage Counselling
  • For Widow/Widower
    • Death Certificate of Deceased Spouse
    • CENOMAR
  • For Foreign Applicants
    • Passport ID
    • Certificate of Legal Capacity to Marry (Foreigners diplomatic official in the Philippines)
    • Divorce Decree (for divorced applicants)
    • Death Certificate if Foreigner is a widow/widower
    • CENOMAR
  • For Marriage under Article 34
    • Man and Woman must have been living together as husband and wife at least five years before marriage

REGISTRATION OF THE CERTIFICATE OF MARRIAGE

  • Requirements:
    • 4 copies of duly accomplished Certificate of Marriage with Signatures of Solemnizing Officers, Contracting Parties and Witnesses.

DELAYED REGISTRATION OF THE CERTIFICATE OF MARRIAGE

  • Requirements:
    • Negative Certificate issued by PSA
    • Certificate of Marriage (if available)
    • If no longer available – affidavit of solemnizing officer stating such facts of marriage
    • Affidavit of marriage executed by the couple stating facts of such marriage
    • Affidavit of 2 disinterested persons
    • ID cards of applicant
    • 4 copies of correctly and completely accomplished certificate of marriage

REGISTRATION OF CERTIFICATE OF DEATH

  • Requirements:
    • 4 copies of Duly Accomplished Certificate of Death from the Hospital.

DELAYED REGISTRATION OF CERTIFICATE OF DEATH

  • Requirements:
    • 4 copies of duly accomplished certificate of death from Hospital.
    • Cause of Death (RHU)
    • Negative Certificate issued by PSA
    • Certificate of Burial Rites from the church/ Barangay Certification
    • Affidavit of Two Disinterested persons
    • Affidavit of Death

LEGITIMATION AND PETITION TO USE THE SURNAME OF THE FATHER PURSUANT TO RA 9255

  • Requirement A.:
    • Notarized joint affidavit of legitimation of parents
    • Certificate of Live Birth in PSA copy
    • Certified marriage contracts of parents
    • Notarized affidavit of paternity if child is not acknowledge by the father
    • Any document showing clearly the paternity between the father and child
      • Employment record
      • SSS/GSIS records
      • Insurance
      • MDR
      • SALN
      • Valid ID of the parents
      • CENOMAR
  • Requirements RA 9255:
    • Certificate of Live Birth in PSA copy
    • Duly notarized affidavit of admission of paternity
    • Duly notarized affidavit to use the surname of the father

ENDORSEMENT OF CIVIL REGISTRY RECORDS TO THE OFFICE OF THE CIVIL REGISTRY

  • Requirements:
    • Letter Request
    • Negative Certificate issued by PSA within the past 6 months.

SUPPLEMENTAL REPORT OF MISSING INFORMATION

  • Requirements:
    • Letter Request
    • Photocopy of Security paper from PSA of the affected civil registry record
    • Duly Notarized supplemental affidavit

ISSUANCE OF COPIES OF CIVIL REGISTRY DOCUMENTS

  • Requirements:
    • Valid ID for document owner
    • Authorization/PSA if not the document owner with valid ID of the document owner.

CORRECTION OF CLERICAL ERROR IN CIVIL REGISTRY DOCUMENTS/ CHANGE OF FIRST NAME/GENDER/DAY & MONTH OF BIRTH (RA 9048 & RA 10172)

  • Requirements:
    1. Change of First Name
      • Certificate of Live Birth from PSA
      • Police Clearance
      • NBI Clearance
      • Certificate of Employment if employed or Affidavit of no Employed
      • Baptismal Certificate
      • Newspaper Publication
      • Any 2 Supporting Documents that justify the name to be changed such as but not limited to:
        • Form 137 Elementary
        • COLB of Children
        • Marriage Contract
        • Voter’s Registration
    2. Correction of Clerical Error
      • PSA issued a document to be corrected
      • Baptismal Certificate
      • Two Supporting Documents to the error to be corrected as required by the MCR (e.g. voters registration/form 173/COLB)
    3. Correction of Clerical error Day, Month & Gender
      • Certificate of Live Birth from PSA
      • Police Clearance
      • NBI Clearance
      • Certificate of Employment if employed or Affidavit of No Employment if not employed
      • Baptismal Certificate
      • Medical Certificate or affidavit of medical records
      • Newspaper publication
      • Medical Certificate issued by MHO w/ certificate authenticity
      • Any 2 supporting Documents that justify the entry to be corrected such as but not limited to (form 137, Marriage contract, Voters Registration, MDR/ Insurance, Driver’s License)

REGISTRATION OF COURT DECREES/ OTHER STATUS OF PERSONS

  • Requirements:
  • 4 Copies of Certified true copy of court decree
  • 4 Copies of Certificate of finality]
  • Certificate of authenticity
  • Certificate of live Live Birth issued by PSA
